Yizkor Book

The Chabad Yizkor Book will be distributed during the High Holiday Yizkor service. The book offers an opportunity for everyone to honor the memories of beloved relatives or friends by including their names in this beautiful, commemorative book. A full page is 5x8 inches. Ads from 1/8 of a page and larger include names as well as a personalized message. A "listing" is the name of your loved one(s) placed in alphabetical order with other names.

Please complete this form and submit it no later than September 17. Credit card charges are accepted. Checks should be made payable to "Merkos Chabad-Lubavitch". Please check over all your text to ensure proper spellings.
